Steps to Request Transcripts and School Records
Update 12/15/2020: Due to the large number of requests being received, at this time turn around time for records/transcripts is 5 business days. Requests are being processed as quickly as possible in the order they are received. We understand the importance of your request and thank you in advance for your consideration and patience as we work to continue to provide services while complying with state and county health and safety protocols.
Due to the State of California Regional Stay-at-Home order for Madera County that went into effect on December 6, e-mail or mail delivery service is recommended and encouraged.
Please see below for options on how to requests records. If you have any questions please call Belen Lopez in the Student Services department at 559-416-5862. If unavailable, please dial 559-416-5858 for immediate assistance. You may also send an e-mail to firstname.lastname@example.org or email@example.com to request information or assistance.
**For third-party/education verification requests: E-mail is the preferred method of processing requests at this time. Please e-mail your request, with a signed release from the individual, to firstname.lastname@example.org or email@example.com. Requests can also be submitted through Parchment.com. Please note that there is a fee when placing your request through Parchment. **
OPTION 1: Place an order for your transcript through Parchment.com.
You will need to create a free account. Electronic transcripts (e-scripts) are free to both current and former students. Electronic transcripts can be sent to yourself, employers, and colleges. Printed copies are free, but there is a mailing fee that starts at 2.50 depending on the shipping speed selected. If printed copies are required, but the shipping fee is a hardship, please contact our office for assistance.
Please note that at this time, Parchment is only for high school transcripts and education verifications. If you need other records, please see Option 2 below.

OPTION 2: For all other records or requests, please follow the instructions listed below:
Instructions to request your school records
- Fill out Request for Student Records form (click on the link to download the form). (Spanish form: Spanish Record Request Form)
- Sign and date at the bottom of the request form
- Send a copy of your ID (Drivers License, State ID, or Birth Certificate)*required if we are not mailing/releasing to an educational institution*
- Scan: email form to firstname.lastname@example.org OR email@example.com
- Fax: 559-661-2005, Attn: Registrar, Student Services
- Mail: Madera Unified School District - Student Services/Registrar - 1902 Howard Road - Madera CA 93637
- There is no cost for the first two copies requested. Please allow up to 5 days to process this request.
Once received, your request may take 3-5 business days to process. Records can be mailed or e-mailed (please note only unofficial records can be emailed). If pick-up is required, please contact our office to make arrangements at 559-416-5862.
For Special Education records, including IEP or 504 copies, please contact the Special Services Department.
Please note: At this time, we cannot reproduce diploma copies. Graduates receive the original diploma upon graduation. An official transcript copy serves as a record of attendance and verification of graduation.
Students currently enrolled can request transcripts and school records from their school site.

Madera Unified has recently adopted an e-scripts transcript service, called Parchment, which allows us to quickly and securely send transcripts electronically to colleges and universities.
There is no cost to current students or alumni for electronic transcript copies. Copies can be printed and mailed for a small fee.
Please note that at this time only transcripts can be requested through this e-script service. If you need copies of other records, alumni should contact the district office at 559-416-5862 (additional information here) and current students should contact the school they are attending. For Special Education records, including IEP copies, please contact our Special Services department.

If you attended Madera Adult School, please read the information below. Not all requests can be processed through Parchment at this time.
For GED Test Records - Years 1966 to 2001 only--Please contact the Madera Adult School directly. There is a $5.00 (Cash/Cashiers Check Only) if requested by the examinee. There is no fee if requested by an institution or agency. Please contact: Madera Adult School @ (559) 675-4425
