Cal Card Pre-Authorization Request Form is used to obtain prior approval to purchase items/services using State and Federal funding with an Administrator District issued Cal Card.

*All State and Federal purchased items must have prior approval from the Director of State and Federal Programs. The form must also include a quote of the items/services being purchased.
