Declaration of Driving Vehicles on District Business;

• If you will not be using your personal vehicle for reimbursement and/or transporting students, select No, fill out Section 2, and sign at the bottom. 
• If you will be using your personal vehicle for reimbursement and/or transporting students, select Yes, fill out Section 1 and 2, fill in your insurance information and submit with a copy of your insurance declaration. (YOUR INSURANCE COVERAGE MUST MEET THE REQUIREMENTS LISTED ON THE FORM)

Driver Authorization Form to use a District Vehicle;

• Fill out completely.

Authorization for Release of Driver Record Information;

• Fill out the top portion only.

We also need a copy of your Driver's History. You may go to the California DMV website and get a copy of your Driver History Report;

Lastly, we will need a color copy of your Drivers' License.
